Behavior-based safety (BBS) has been around since the 1980s, but the way it is run in 2026 looks nothing like the paper SOR (Safe Observation Report) card programmes most organisations still maintain. ISO 45001 §5.4 now treats worker participation as a clause-level requirement, OSHA VPP recertification reviews scrutinise leading indicators as closely as TRIR, and EU-OSHA's Vision Zero campaign pushes near-miss reporting from "nice to have" to "expected baseline".
If your near-miss programme captures fewer than 1 observation per FTE per month, or your near-miss-to-recordable ratio is under 10:1, you are not measuring safety — you are measuring how comfortable your workforce is with reporting. This playbook covers what a defensible 2026 BBS programme looks like, what to measure, and how to roll one out in 90 days.
1. The 2026 safety-culture landscape
Three forces reshaped BBS expectations between 2020 and 2026:
- Regulatory shift to leading indicators. OSHA's enforcement guidance now expects leading-indicator data in VPP applications and recertifications. - Observations of unsafe acts do not feed performance reviews. Ever. This must be policy, communicated, and union-endorsed where relevant.
- Supervisor coaching is the unit of work, not the observation count. Measure supervisors on coaching conversations held, not cards collected by their team.
- Stop-work decisions get visible recognition. A worker who stops a job should be publicly thanked by name, even (especially) when the stop turns out to be a false alarm.

- Cash bounties for hazard finds — turns safety into piecework.
The rule of thumb: recognise behaviour (engaging with the programme, coaching peers, stopping work) not output (number of cards).
8. Integration with incident management & corrective actions
A BBS programme that does not connect to incident management is a parallel dead-end inbox. The integration loop has three legs:
- Escalation: Every observation can be promoted to a near-miss; every near-miss can be promoted to a recordable incident — without re-keying data.
- CAPA close-out: Corrective actions created from observations are visible in the same action tracker as incident actions. One inbox, one SLA.
- Investigation evidence pull-through: When an incident is investigated, related observations from the same site, job or behaviour cluster are surfaced automatically as context.

- Weekly retrospectives with site leadership.

10. A simple KPI dashboard template
A monthly leading-indicator dashboard for a multi-site programme typically has five panels:
- Participation heatmap — sites × months, colour-coded by % of frontline submitting.
- Observations per FTE trend — 12-month rolling, by site, with target band.
- Near-miss-to-recordable ratio — 12-month rolling, by site.
- Top 10 at-risk clusters — from AI clustering, ranked by frequency × severity, last 30 days.
- Action close-out SLA performance — % closed within SLA, by site and supervisor.

11. FAQ
Should observations be anonymous? Both — configurable per category. Anonymous for stop-work and concerns; attributed for routine observations to enable coaching and recognition. Anonymous-only flattens the coaching loop; attributed-only suppresses the most safety-critical reports.
How do I avoid card-stuffing? Cap individual contribution to team scores, weight quality over count, audit cards for content quality monthly, and never tie individual leaderboards to financial rewards. The strongest control is cultural — make it clear that the programme is judged on outcomes, not throughput.
What if union/works-council resist? Engage them in the charter. Make the no-discipline policy explicit, give them visibility into anonymisation handling, and offer joint review of the leading-indicator dashboard. Most resistance is to disciplinary use of observations — eliminate that risk and most concerns dissolve.
Do we still need TRIR/LTIFR? Yes. Leading and lagging indicators are complements, not substitutes. Drop lagging KPIs and you lose the ability to verify that the leading programme is actually changing outcomes.
How long until we see TRIR move? 12–24 months is realistic when the leading programme is paired with CAPA close-out. The first 6 months will likely show TRIR up as underreporting unwinds — plan for that signal and communicate it to leadership in advance.
What is a good size for the programme team? 1 programme lead per ~3,000 frontline users, plus distributed site champions (typically 1 per 200–500 workers). Software replaces coordination work; it does not replace the human work of coaching and recognition.
