    Final field-verification before introducing hazardous chemicals: construction matches design, P&IDs are accurate, all MOC actions are closed, operators are trained, and safety systems are functional. Required by OSHA PSM 1910.119(i).

    PHA — Process Hazard Analysis

    Systematic team-based review (commonly HAZOP) that walks each process node through deviation guidewords (No/More/Less/Reverse/As-well-as) to identify causes, consequences, existing safeguards, and recommended actions. Required every 5 years under PSM.

    LOPA — Layer of Protection Analysis

    Semi-quantitative scenario analysis that takes a PHA finding, multiplies the initiating event frequency by the probability-of-failure-on-demand of each Independent Protection Layer (BPCS, alarms, relief, SIF), and compares to a tolerable target — driving the required SIL.

    Hot Work + Energy Isolation Permit

    Combined high-risk permit for upstream/midstream/downstream — atmospheric gas testing (LEL/O₂/H₂S/benzene), positive isolation list, blind register, fire watch, simultaneous-operations review, and dual sign-off. Replaces ad-hoc work permits with a single auditable record.
