PHA vs LOPA: When to Use HAZOP, When to Layer LOPA on Top
Most teams confuse PHA and LOPA. They are not alternatives — they are sequential. Here is exactly when each applies, what they output, and how to scope a study without burning weeks of engineering time.

What PHA (HAZOP) Does
A HAZOP team walks each process node through deviation guidewords:

For each deviation: causes → consequences → existing safeguards → recommended actions.
Output: a qualitative ranked list of scenarios with recommendations.
When to Layer LOPA on Top
Apply LOPA only when:
- PHA flagged the scenario as high consequence (fatality, major release, asset loss)
- Existing safeguards are credited as Independent Protection Layers (IPLs)
- You need to decide whether a Safety Instrumented Function (SIF) is required and at what SIL
LOPA math (simplified):
Mitigated frequency = Initiating event frequency × ∏ (PFD of each IPL)
Gap = Mitigated frequency / Tolerable frequency
If the gap > 1, you need additional IPLs (typically a SIF) to reach the tolerable target.
