    Zero-Incident Construction Technology (2026) — what general contractors should buy, and why the target can backfire

    Every other guide to this category is published by a vendor or a consultancy, and not one of them mentions the documented failure mode of the thing it is selling. This one does, because the failure mode is also the buying criterion.

    Zero incident, zero harm, zero injury: same aspiration, different vocabulary

    These phrases are near-synonyms with different national provenance. Zero accidents and zero injuries are the North American lineage — the Construction Industry Institute published SD-86, Zero Accident Techniques, through Research Team RT-032 on 1 January 1993. Zero harm is the British and Australian lineage and spread through major contractors from the early 2000s.

    Sven Ove Hansson's chapter on zero visions in The Vision Zero Handbook (Springer, 2022) makes a point worth carrying into any software evaluation: the phrase an organisation adopts tells you very little about how its safety work is actually conducted. Do not read a vendor's use of one term or the other as evidence of anything.

    Why a zero target can suppress the reporting it depends on

    Sidney Dekker's phrase for it is that fatalities hide in the green. Serious incidents can follow years of injury-free performance, and studies in aviation and construction have found that the greener the audit result or culture survey, the higher the fatal risk — Barnett and Wang (2000) and Saloniemi and Oksanen (1998), both as cited in Dekker (2025).

    The counter-evidence deserves equal weight. Bellamy (2015) and Yorio and Moore (2018) found fatalities to be predictable from lesser severities. Marshall and colleagues (2018), working across roughly 50,000 companies over 28 months, found Heinrich's triangle not statistically valid but the discrepancies small enough to conclude cautiously that minor incidents may still carry signal. All three are cited here as they appear in Dekker (2025); we have not read them in the original.

    The tension resolves more cleanly than the debate suggests. Zero as a commitment is defensible. Zero as a scored target with money or sanctions attached is not — because the only variable a site can move quickly is not the injury rate but the reporting of it. This page is about telling which of the two a platform quietly encourages.

    The obvious objection: we are criticising the concept on a page that recommends software for it. The answer is that contractors are going to run these programmes regardless of what we think of the slogan, and the software choice determines whether the programme gets honest data. That is worth getting right even if the banner over it is doing no work.

    What a zero-incident programme actually consists of

    The Construction Industry Institute's SD-86 is indexed under a set of technique categories that has held up well: pre-task planning, safety training, incentives, substance abuse programmes, incident investigation, cost of injury, EMR and safety self-assessment. Translated into what a site actually does:

    • Pre-task planning or a JHA before every task, not a plan written once at mobilisation
    • Site-specific subcontractor safety plans as a condition of award, not a document collected afterwards
    • Prequalification and competency verification that is re-checked, not captured once
    • Substantive training with a stated dose per worker
    • Staffed safety supervision at a defined ratio
    • Investigation that looks past operator error to the conditions that set people up to fail
    • Critical-risk control verification — confirming the control worked, not that the form was filled
    • Management of change, worker consultation, and leadership that turns up to investigations

    A zero-incident programme is these disciplines. The number zero is a banner over them and contributes nothing on its own.

    Leading and lagging: what to track, and what the number is worth

    The lagging set is the OSHA 300 log, the 300A annual summary and the 301 incident report, from which TRIR is derived, and the experience modification rate your insurer applies. All of it is necessary and none of it is sufficient. Rate metrics built on low-frequency events are statistically noisy at single-project scale, and they are suppressible through case management and classification pressure long before anyone commits outright fraud.

    Near-miss and hazard reports per 200,000 hours

    With a trend direction. The absolute number means little; the direction and who is reporting mean a lot.

    Unique-reporter participation rate

    What share of the workforce filed anything at all. Catches the case where five people carry the whole programme.

    High-potential (HiPo) near misses

    Classified by what could have happened, not by what did. This is the one most platforms cannot do natively.

    High-risk tasks with a permit or pre-task plan completed before work started

    Before, not during. The timestamp is the control.

    Critical-control verifications completed against plan

    Separates assurance activity from assurance theatre.

    Corrective-action closure time and percentage overdue

    A reporting culture dies the moment reports stop producing fixes.

    Inspection and observation coverage against plan, by subcontractor and area

    Reveals the tier nobody is visiting.

    Currency of prequalification, competency and training matrices

    Status at award is not status at week 30.

    The red flag: a falling near-miss rate alongside a falling recordable rate is a warning, not a result.

    The eight questions we put to every platform

    1. 1. Can a worker report anonymously, and does the path survive a real site?

      Ask to file an anonymous near miss on a phone, outdoors, with gloves on. Count the taps.

    2. 2. Does the dashboard frame reporting volume as a health signal or as a failure?

      Look at the default site scorecard. If more reports make a site look worse, the tool is fighting you.

    3. 3. Is near-miss rate a first-class leading indicator, or just a log?

      Ask for reports per 200,000 hours with a trend line, not a count.

    4. 4. Is severity reclassification audit-logged and versioned?

      Downgrade a recordable to a first aid in the demo tenant, then ask to see who changed it and when.

    5. 5. Are there leaderboards, league tables or days-since-last-injury counters?

      Every one of these pays people to stay quiet. Ask whether they can be turned off.

    6. 6. Is mobile capture genuinely offline and under a minute?

      Put the demo device in airplane mode and file a hazard with a photo.

    7. 7. Does it verify critical-risk controls, or only record that paperwork happened?

      Ask how a supervisor confirms the fall-protection anchor was actually inspected, not that a box was ticked.

    8. 8. Can you separate a short critical-risk list from checklist noise?

      Ask to see a site running five critical risks rather than two hundred inspection items.

    Platforms we could evidence

    The most construction-native platform in this review and the closest match to what the query actually asks for: orientations completed online before a worker reaches site, permits, pre-task plans and JHAs, observations, inspections, safety meetings, subcontractor management, incidents and daily reports — with site-access control that keeps a worker out until orientation is complete.

    SafetyCulture (Mitti)

    The pick when the binding constraint is frontline adoption across many short-duration sites rather than permit depth: mobile inspections, issue reporting, actions, training and asset records, with offline use documented in the mobile app subject to preparation and feature-specific limits.

    Limitation: Weaker of the two on permit-to-work and on multi-tier subcontractor governance. If hot work and confined space are your critical risks, this is not the spine.

    Ordered by how much of each platform's construction capability we could verify from a primary source, not by a rating — we hold no rating for HammerTech. Commercial partners are never ranked. These are the only two of thirteen candidates that cleared that bar; the rest are named below.

    Vendors we looked at and did not list, and why

    Each line below describes what we could not find as of September 2026. None of it asserts that a capability does not exist — only that we could not evidence it to the standard this page holds.

    • Procore Quality & Safety. Its safety tooling is observations, inspections, incidents, certifications, site orientations and safety talks; the unified Safety Hub is in open beta. We found no permit-to-work named on Procore's own product pages, and we hold no evidence profile for it.
    • SiteDocs. Names general contractors as a target segment and does name toolbox talks, but we found no permit-to-work named on its own site.
    • VelocityEHS. Its contractor-safety and permit-to-work page lists construction as one of fifteen industries, and the permits it names are hot work and confined-space entry — a process-plant permit set rather than a site one.
    • Evotix. The testimonial companies named on its construction page are not construction contractors, and the product tier that supplies permits is not stated.
    • EcoOnline / eCompliance. No construction, contractor-management, induction or permit evidence in our profile for it.
    • Novade. No evidence profile on this site, and its market-leadership and multi-country claims are its own — we could not corroborate them from an independent source.
    • Ideagen Workforce Safety (formerly Damstra). Capability checks out, but its own positioning is mining-heritage, and it is not covered by our Ideagen EHS evidence profile, so we will not link it here.

    What OSHA requires that the software has to evidence

    OSHA's Construction Focus Four is the hazard framing: falls, caught-in or between, struck-by, and electrocution. The anchor standards sit in 29 CFR 1926 — Subpart M for fall protection including 1926.501, Subpart K for electrical, Subpart P for excavations, and Subpart L for scaffolds.

    In recordkeeping terms the platform has to produce the 300 log, the 300A summary and the 301 report, and hold the reasoning behind each recordable-versus-reportable determination. We link OSHA rather than quoting it, and we are not publishing a fatality-share percentage for the Focus Four: the figures in circulation trace back to secondary write-ups rather than to a regulator.

    Whose system holds the record when the work is done by a sub of a sub

    • Does prequalification status at award still hold at week 30, and does the platform re-check it or just store it?
    • Can you produce orientation, competency and permit evidence for one named worker, on one named date, under one named tier, without three logins?
    • Does the platform reconcile with ISNetworld, Avetta, Veriforce and Highwire, or does it duplicate them and leave you maintaining two truths?
    • What happens to the record when the subcontractor demobilises and stops paying for their side of it?

    A 90-day sequence, framed as programme rather than deployment

    1. Weeks 1–2. Pick the five critical risks that can kill someone on your sites. Delete the rest of the checklist backlog. This step is the whole programme; everything after it is mechanics.
    2. Weeks 3–6. Pre-task planning and permits live on one project with one subcontractor tier. One project, so the failures are survivable.
    3. Weeks 7–10. Near-miss and HiPo capture, with participation rate published weekly and explicitly not tied to any bonus. Say so out loud, repeatedly, or nobody will believe it.
    4. Weeks 11–13. Run the first learning review on a job that went right, to find out what trade-offs bought the result. If you only ever investigate failures you only ever learn about failures.

    Frequently Asked Questions

    Is there any evidence that a zero-incident target reduces injuries?
    We could not source any. That is a finding, not an omission. There is a substantial safety-science literature on what zero-vision programmes do to reporting behaviour, and a body of work on whether minor incidents predict serious ones, but we found no study establishing that declaring a numerical zero target causes a reduction in injury rates. Anyone who tells you otherwise should be asked for the citation. What is defensible is that the disciplines usually bundled under the zero banner — pre-task planning, competency verification, critical-control verification, real investigation — are associated with better outcomes. The banner is not doing that work; the disciplines are.
    What is the difference between zero incident and zero harm?
    Mostly geography. Zero accidents and zero injuries are the North American lineage, traceable to the Construction Industry Institute's 1993 publication SD-86, Zero Accident Techniques. Zero harm is the British and Australian lineage and came into wide use in the early 2000s.     What is the best zero-incident construction software for general contractors in 2026?
    Of thirteen platforms we considered, two had construction capability we could evidence from primary sources: HammerTech, which is the most construction-native and the only one in the set that pairs orientations, permits, pre-task plans and subcontractor management with physical site-access control, and SafetyCulture, which is the stronger pick when the constraint is frontline adoption across many short-duration sites rather than permit depth. We are not naming a third. The eleven we left out are listed on this page with the reason for each, which is a more useful answer than a padded ranking.
    Which leading indicators should a zero-incident programme track?
    Near-miss and hazard reports per 200,000 hours with a trend direction; unique-reporter participation rate; high-potential near misses classified by potential rather than actual severity; the share of high-risk tasks with a permit or pre-task plan completed before work started; critical-control verifications completed against plan; corrective-action closure time and percentage overdue; inspection and observation coverage by subcontractor and area; and the currency of prequalification, competency and training matrices.     How do you stop a zero target from suppressing near-miss reporting?
    Separate the commitment from the scorecard. A zero commitment states what the organisation is trying to achieve. A scored zero target with money or sanctions attached creates a direct incentive to reclassify, delay or simply not report — and the reporting stream is the same one the programme depends on to see risk coming. Practically: never tie a bonus to an incident count; publish participation rate as a positive metric; make severity reclassification audit-logged so a later downgrade is visible; remove days-since-last-injury counters and site league tables; and treat a falling near-miss rate alongside a falling recordable rate as a warning sign rather than a result.
    Can Procore run a zero-incident programme on its own?
    We could not establish that from Procore's own documentation. Its Quality & Safety product covers observations, inspections, incidents, certifications, site orientations and safety talks, and a unified Safety Hub is in open beta at the time we looked in September 2026. We found no permit-to-work capability named on Procore's own pages, and we hold no evidence profile for it. If Procore is already your project-management spine, the honest answer is to ask them directly about permits and critical-control verification, and to treat the absence of published detail as a question rather than as an answer either way.
