Total Recordable Incident Rate (TRIR) is the single most-watched safety metric in EHS. Boards ask about it. Clients require it on prequalification questionnaires. Insurers price your premium based on it. And yet, half the safety teams I meet are calculating it wrong or comparing themselves to the wrong benchmark.

What Is TRIR?
Total Recordable Incident Rate (TRIR) is a normalized measure of work-related injuries and illnesses per 100 full-time employees per year. It lets you compare your safety performance against companies of any size, in any industry.
The formula:
TRIR = (Number of OSHA-recordable incidents × 200,000) ÷ Total hours worked
The 200,000 comes from 100 employees working 40 hours/week × 50 weeks. It is the normalization constant that makes companies of different sizes comparable.
Worked Example
Your company logged 12 OSHA-recordable incidents over the past year. Your 250 employees worked a combined 520,000 hours.
TRIR = (12 × 200,000) ÷ 520,000 = 4.62
That number alone is meaningless. Is 4.62 good? Bad? Average? It depends entirely on your industry — see the benchmarks below.
What Counts as "Recordable"?
This is where most calculations go wrong. Not every incident is OSHA-recordable. Per OSHA 29 CFR 1904, an incident is recordable if it results in any of:
- Death
- Days away from work
- Restricted work or transfer to another job
- Medical treatment beyond first aid
- Loss of consciousness
- A significant injury or illness diagnosed by a healthcare professional
First aid does not count. Bandaids, ice packs, and over-the-counter medication are first aid, not recordable.
Common misclassifications:
- A worker who twists an ankle and goes home with no medical visit → not recordable
- A worker who twists an ankle and is put on light duty for 3 days → recordable (restricted work)
- Same worker, sees physician for prescription anti-inflammatory → recordable (medical treatment)
What Is LTIR?
Lost Time Incident Rate (LTIR), sometimes called DART (Days Away, Restricted, or Transfer), measures only the more serious cases:
LTIR = (Lost-time incidents × 200,000) ÷ Total hours worked
A "lost-time incident" is one where the worker missed at least one full day of scheduled work after the incident date.
LTIR is always lower than TRIR. The ratio between them tells a story: a TRIR of 5.0 with an LTIR of 4.5 suggests minor incidents are escalating to lost time — a sign of weak return-to-work programs or poor early intervention.

The Bureau of Labor Statistics publishes incident rates annually. Here are the latest benchmarks for major industries:

The 7 Levers That Actually Move TRIR
In my conversations with EHS leaders who reduced TRIR by 40%+ over three years, these seven levers come up again and again:
1. Leading indicator obsession
Lagging indicators (incidents) tell you what already broke. Leading indicators (observations, near misses, audit findings) tell you what is about to break. Top performers track 3-5 leading indicators per site weekly. See our leading indicators guide for which to pick.
2. Near-miss reporting culture
Companies with high near-miss reporting rates have lower TRIR. The math: if you fix a near miss, you prevent the recordable. Aim for a 10:1 near-miss-to-recordable ratio as a target. Most companies are at 2:1 or worse. See our near-miss reporting software guide.
3. Standardized incident investigation
A weak investigation produces a weak corrective action, which fails to prevent recurrence. Invest in 5-Why or TapRoot training for supervisors. Track corrective action close-out rates as a KPI in itself.
4. Behavior-based safety observations

7. Frontline ownership
The biggest predictor of TRIR reduction is whether frontline supervisors own safety in their crew, or whether it is "the EHS guy's job." Common TRIR Calculation Mistakes
- Including first-aid cases — inflates TRIR artificially.
- Excluding contractor hours but including contractor incidents — distorts the rate.
- Using calendar hours instead of actual hours worked — overtime, sick leave, and vacation skew the denominator.
- Annualizing partial-year data incorrectly — TRIR is a 12-month rolling rate, not a Q1 number multiplied by 4.
- Cherry-picking the comparison benchmark — comparing yourself to "all industry average" when you are a healthcare provider is misleading. Use NAICS-specific benchmarks.

Is TRIR per 100 employees or per 200,000 hours?
Same thing. The 200,000 multiplier represents 100 full-time employees working a year (100 × 40 × 50 = 200,000). The rate expresses incidents per 100 FTEs.
What's a "good" TRIR?
Below the top-quartile benchmark for your industry (see table above). For most industries, sub-1.5 is excellent.
Should I use TRIR or LTIR for my safety KPI?
Both. TRIR captures all recordable injuries; LTIR captures severity. Tracking both prevents you from gaming one at the expense of the other.
How often should I calculate TRIR?
Calculate a rolling 12-month TRIR monthly. This smooths out short-term noise while keeping the metric current. Comparing single-month rates is meaningless — they're too volatile.
What's the difference between TRIR and OSHA Incidence Rate?
They are the same metric. "OSHA Incidence Rate" is the formal name; "TRIR" is the industry shorthand. Both refer to the recordable incident rate per 200,000 hours worked.
