    Kizeo Forms Pricing

Kizeo Forms lists 4 published prices on its own pages, from USD 18 to USD 28 per user, per month (annual billing).

Published prices, checked 2 October 2026. List prices, not quotes; totals are arithmetic on them. How prices are recorded

    How Kizeo Forms charges

Kizeo Forms charges per user per month in USD on its US page: Smart USD 18 billed annually or USD 25 monthly, Business USD 28 or USD 40. All users in one account must use the same plan, so the whole account is bought on one tier.

Other regions may use different currencies and rates.

Because one account must be on one plan, the cheaper tier applies only if no user needs a Business feature. A single user who needs AI voice/photo entry or BI features moves every seat to Business, so compare an all-Smart total with an all-Business total, not a blend of the two.

Both tiers are cheaper billed annually (Smart USD 18 against USD 25 monthly, Business USD 28 against USD 40). The monthly premium is larger on Business, so the choice of term matters most where Business is bought.

The amounts are those on the US page. A buyer elsewhere should not budget from them without a regional quote, and neither a minimum seat count nor the tax treatment was established, so the team totals on this page are the listed rate times the users entered and nothing more.

The record does not say whether annual billing means a prepayment, a twelve-month commitment or both, and the entry figure of USD 18 assumes annual billing. Ask what a customer on the annual rate can change mid-term, such as adding users or moving a tier, and what a monthly customer gives up by not committing.

What is established is narrow: the US per-user amounts and the rule that every user in an account shares one plan. The review leaves regional terms and minimum seats to the quote, so a buyer should treat support, onboarding and any seat floor as items to be written into the proposal, not as things the public page already answers.

Advanced input and BI are the features that decide the plan. If only a few people need AI voice or photo entry, the one-plan rule means they set the price for everyone, so list each role and the feature it truly needs before comparing Smart and Business totals.

Smart includes offline access and API, so the entry price is not a tier that leaves those two out. The record cautions, though, that a general offline statement does not show how conflicts or interrupted synchronisation are handled, so having the feature is not yet proof that it meets your recovery needs.

What each Kizeo Forms plan covers, as recorded

    Smart
    Includes offline access and API.
Not included: AI voice/photo entry and BI features, which are added in Business.
    Business
Includes AI voice/photo entry and BI features, added to Smart.
    Offline capture
Offline access is part of the entry tier, so it does not require Business. A general offline statement does not establish offline AI processing or guaranteed recovery after an interrupted synchronisation.
    API
Included from Smart. The pricing text gives no call limit or connector list, so integration capacity is a question for the quote.
    Reporting and BI
BI features are listed only in Business. A buyer whose need is a report delivered to another record system should test that delivery on the tier under consideration.
    One plan per account
Stated as a rule for users in one account. Whether the users could be split across accounts, and what that would do to administration and shared forms, is not stated, so ask instead of assuming.

What to ask Kizeo Forms to put in the quote

Questions prepared by The QHSE Standard from the recorded terms and gaps; they are not statements from Kizeo Forms.

    • The minimum paid-seat quantity and the tax treatment; neither was established.
    • Whether the account can mix user roles without buying unused advanced features, given the one-plan rule.
    • Regional terms, if you buy outside the US.
    • Offline capture and successful synchronization after an interrupted connection.
    • Report output and delivery to the record system you already use.
    • How AI-generated values are reviewed and corrected before submission.
    • Advanced input and BI requirements change the plan needed, and offline conflict and recovery details need a device-level demonstration.
    • Three quotes side by side: every user on Smart, every user on Business, and your preferred split, so the cost of the one-plan rule is visible before signing.
    • What happens after a form is submitted and who owns the next step: the record cautions to validate workflow ownership and reporting requirements beyond data capture.
    • A written position on offline AI: do not assume voice or photo entry works without a connection because the app has an offline mode.
    • The API allowance in Smart in practice: what it can read and write, any limits, and whether the connector your record system needs is covered.
    • Every answer the vendor gives on regional terms and minimum seats, written into the order form: the review checked only the US amounts.
    • The tier that fits your hardest requirement for AI-filled values and reporting, shown on your own form, with the correction step before submission.
    • Written confirmation of the plan each feature on your shortlist belongs to: offline access and API are listed in Smart, while AI voice/photo entry and BI features are added in Business.
